Leading questions replace the interviewee's reality with the interviewer's model. A single word change can shift reported data threefold — subjects asked about 'frequent' headaches report three times as many as subjects asked about 'occasional' headaches. Questions must pull information out, not push assumptions in.
Example
'How much of a problem is documentation for you?' assumes documentation is a problem. 'What does your documentation process look like after a procedure?' does not.
